Enrollment

We are currently accepting enrollment for all grades Kindergarten through 8th for 2017-2018 school year.  New students to Kindergarten or the Chicago Public School district, as well as transfers, need to complete all the forms and provide the additional documents on the registration checklist.

  • Student Pick-Up Authorization Form
  • Media Consent form
  • Home Language Survey
  • Medical Information Survey– Allergies
  • Oral Exam Consent form – only if your child[ren] haven’t had a dental exam for the year 2018 or are entering Kindergarten, 2nd and 6th
  • Emergency Information and Contact Form
  • Family Income Information Form (required by every household)

Additional Required Documents

  • Transfer/Release Form (if transferring from previous school)
  • Birth Certificate
  • Health & Immunization Forms
  • Final Report card and/or standardized test scores

Two (2) Proofs of Residency

  • Drivers License
  • State I.D.
  • Voter’s Registration
  • Utility Bills (electric, gas, water)
  • Cable, Insurance, Cell Phones, Credit Card Statements
  • Rental Lease

Dress Code – Everyday students are required to wear

  • Black shoes
  • Black pants or knee length skirts (approximately 1 inch above the knee)
  • Green or Red Kwame Nkrumah Academy polo shirt (at least 1)
    • You may purchase extra plain red or dark green polos at any retailer
  • Black or blue solid cardigan sweaters  (no stripes, bold lettering, or logos)
  • Returning students  may continue to wear gold polos

       For Physical Education

  • Students may wear a KNA sweatsuit the day of gym
  • KNA gym uniform – black shorts and black t-shirt with KNA logos
  • Plain black shorts and black t-shirt – no stripes, bold lettering, or logos)

Physical Examinations are REQUIRED if your child(ren) is/are:

  • entering Kindergarten or 6th
  • entering the Chicago Public School district for the 1st  time or transferring back into the district
